Home Care Services

(HHA and CNA)

JFS at Home services enable seniors to live with dignity and independence, in the comfort of their own home, with a customized level of care. An RN who specializes in home care will establish the right care plan and supervise care on an ongoing basis.

Services include: Light Housekeeping, Bathing and Showering, Dressing and Undressing, Assistance with Personal Hygiene, Toilet Hygiene, Meal Preparation, Transportation to Medical Appointments and Assistance with Social Activities (i.e. shopping, movies, visiting with friends).

Caring for Individuals with Cognitive Impairment

Alzheimer’s disease and Dementia are the most common conditions that affect an individual’s intellectual and social abilities. Cognitive impairment causes disruption in a person’s ability to function independently. Such conditions can take a significant toll both emotionally and mentally on the afflicted person as well as those close to them.

JFS at Home caregivers are trained in providing compassionate and appropriate care for individuals experiencing cognitive impairment. A safe, familiar environment is often the most comforting place to be for an individual experiencing the effects of these conditions. Caregivers conscientiously monitor behaviors and redirect clients using proven techniques that maintain our clients’ dignity and well-being. Our caregivers will address the mental, emotional, and physical needs of your memory impaired loved-one while monitoring their environment for safety.

Medication Reminders

According to the Centers for Disease Control and Prevention (CDC), most adults are currently taking at least one medication while nearly one-third of older adults take more than four prescription medications at the same time. There are a growing number of adults, approximately four out of five, who will need reminders to take their medications. Remembering to take medications on time is a big challenge, especially for the elderly, JFS at Home caregivers can remind clients when to take their medications, help them read labels, open medication containers or pill boxes, and order medication re-fills. This type of assistance helps the client maintain a consistent and timely medication regimen, preventing misuse of medications or medication errors by the client.

Mobility Assistance

 (Walking, Transferring from bed / chair/ car, etc.)

JFS at Home caregivers assist clients with their mobility needs to reduce falls and allow for safe ambulation within and outside their home. Caregivers are trained to assist clients with use of a cane, walker, risers for chairs or couches, swivel seat cushions for the car, and wheelchairs. Our caregivers are trained to supervise clients for safety around their home and to assist by decreasing clutter and clearing passageways.

At-Home Assessments

An RN will conduct an in-home assessment. To create an individualized care plan, the RN will:

  • Review the medical history and medications
  • Assess ambulation and mobility within and outside the home
  • Assess safety with transfers in and out of the shower as well as for toileting
  • Make recommendations for home safety devices (DME) that assist in overall comfort and safety such as walkers, canes, wheelchairs, handrails, and shower chairs
  • Assess the layout and floor plan of the home with suggestions on how to improve safety and reduce falls in each area of the home

Respite Relief for Family Caregivers

Respite care provides primary caregivers with a temporary break from caregiving. This important relief allows family caregivers to recharge and return refreshed and better prepared to face the continued challenges of caregiving.
Respite care provides caregivers the opportunity to spend more time with family and friends, and run errands.
