JFS AT HOME ABOUT US

Quality Care at Home

What distinguishes JFS at Home from other homecare agencies is the long history of trust and respect that Ruth & Norman Rales Jewish Family Services (JFS) has established throughout the community. That reputation is based on over 35 years of providing programs and services with integrity, compassion, social responsibility and humanitarian values JFS at Home, offers: Companion Services and Non-Medical Private Duty Home Care Services, and Geriatric Care Management.

JFS at Home provides quality home care for seniors, allowing them to continue living at home where they feel most comfortable and enabling them to have the greatest level of wellness, dignity and independence throughout life. While home care is often a welcome alternative to a nursing home or assisted living facility, JFS at Home provides care wherever you call home.

Our Mission

“To promote healthy aging by enabling clients to maintain their quality of life while they continue to live at home, safely and with dignity. We reinvest a portion of our profits to make these services affordable for those in need.”

OUR VISION

JFS at Home will offer our clients a highly coordinated and caring experience, distinguished by the quality of people associated with our agency. Through excellence and innovation in client care, we intend to become a leader in the home health care industry.

It is sometimes hard to determine the level of care needed for you or your loved one and it can seem impossible at times with so many agencies out there. JFS at Home wants to make this an easy process for you and we will guide you along the way to choose the options that best fit your needs.

From your first phone call to starting services, we have simplified the process into 4 steps

1

Contact us for a free phone consultation. Share as much as you can about your needs, schedule and situation in order to put the process into action.

2

A Registered Nurse (RN) or Licensed Clinical Social Worker (LCSW) will be sent to your loved one wherever they live for an in person assessment to best understand their needs and level of independence with activities of daily living. We also want to get to know them on a more personal level to match them with their caregiver. ($100 fee for this assessment)

3

Assessment and requested schedule are shared with the JFS at Home team and we work together to identify the best caregiver to meet your unique needs.

4

A  Scheduling Coordinator will contact you to provide the name of your caregiver and to review the schedule with you.

Rafie Merzel

Registered Nurse

Rafie Merzel has been with JFS at Home since 2016 working in the capacity of an RN. In her thirty-six years of practice as a nurse, Rafie has worked in many areas of healthcare. Early in her career she worked on a medsurg floor as well as on a respiratory unit with ventilators and traches, then progressed to Geriatric-Longterm rehab Care due to her love of the elderly population and wanting to make a difference in peoples senior years. She then found her niche in rehab and home-care as it proved to be very unique, enabling her to have intimate contact with patients and clients, which satisfied her desire to teach and support them through some of their most challenging times of life experiences. What drew her to this particular field of nursing was the challenge it affords her in her working day, as “No two patients/clients” are exactly alike and “no two days are the same.” Nothing gives a nurse more pleasure than to see someone overcome their medical, physical and emotional challenges and resume their quality of life to a healthy, functional independent state. Rafie received her nursing degree from Farmingdale/ Adelphi Universities in New York. In her free time she loves to garden, soak up the sun, travel and spend quality time with her spouse and her dog. She continues to strive to continue her learning and educational skills/studies by pursing future degrees.
